SP6 Fred M. Kuddes, Jr. was killed on 27 January 1967. SP6 Kuddes was assisting with some experimental work at Dugway Providing Ground, Utah, when he was killed. SP6 Kuddes was flying in a Piasecki CH-21C Shawnee helicopter from which E158 CS (tear gas/riot control agent) canister cluster were being dropped. Once of the clusters onboard the aircraft functioned, setting the helicopter on fire and filling its interior with CS. The helicopter crashed killing SP6 Kuddes.
